PSVD is still often misdiagnosed as cirrhosis, emphasizing the need to improve PSVD diagnosis strategies. Data on liver stiffness measurement (TE-LSM) using transient elastography, in PSVD are limited. The aim of this study was to evaluate the accuracy of TE-LSM to discriminate PSVD from cirrhosis in patients with signs of portal hypertension.
Retrospective multicenter study comparing TE-LSM in patients with PSVD, according to VALDIG criteria, to patients with compensated biopsy-proven cirrhosis related to alcohol (n=117), hepatitis C virus (HCV) infection (n=110) or non-alcoholic fatty liver disease (NAFLD) (n=46). All patients had at least one sign of portal hypertension among gastroesophageal varices, splenomegaly, porto-systemic collaterals, history of ascites or platelet count &lt; 150 x 10/L. The 77 patients with PSVD included in the test cohort had lower median TE-LSM [7.9 kilopascals (kPa)] than the patients with alcohol-, HCV- and NAFLD-related cirrhosis (33.8 kPa, 18.2 kPa, and 33.6 kPa, respectively; p&lt;0.001). When compared to cirrhosis, a cut-off value of 10 kPa had a specificity of 97% for the diagnosis of PSVD, with a 85% positive predictive value. A cut-off value of 20 kPa had a sensitivity of 94% for ruling-out PSVD, with a 97% negative predictive value. 94% of the patients were well-classified. Even better results were obtained in a validation cohort including 78 PSVD patients.
This study including a total of 155 patients with PSVD and 273 patients with cirrhosis demonstrates that TE-LSM &lt;10 kPa strongly suggests PSVD in patients with signs of portal hypertension. Conversely, when TE-LSM is &gt;20 kPa, PSVD is highly unlikely.

Although Pickering emulsions have been reported as presenting greater stability when compared with surfactant-stabilized emulsions, little is known about the drying stability of Pickering emulsions (which is important for film applications). Anyway, several studies have indicated that Pickering emulsions are interesting systems to improve the water vapor barrier properties of bio-based films and coatings, and to act as carriers of active hydrophobic components. On the other hand, the tensile properties of those films are usually impaired by the presence of Pickering emulsions. Multicentre (n=3 UK referral clinics), retrospective, observational study. Database searches were performed (2007 to 2019) to identify dogs with detrusor urethral dyssynergy. Dogs with structural abnormalities or detectable neurological disorders affecting micturition were excluded. Clinical presentation, diagnostic procedures, treatment and outcome were evaluated.
Thirty-five dogs were included. Middle-aged, large-breed, male neutered dogs were most frequently documented. Four female dogs were included. Fifteen breeds, including Labrador retrievers (8/35; 22.9%), golden retrievers (5/35;14.3%) and cross-breeds (5/35; 14.3%) were identified. Median duration of clinical signs was 152?days (range 0 to 1095). All dogs were dysuric at presentation with 17/35 (48.6%) reported to have an altered stream of urine and 17/35 (48.6%) to be stranguric. Follow-up data were available for 34 dogs (median 136?days, range 4 to 2188). Response was classified as good (20/34; 58.8%), partial (7/34; 20.5%) or poor (7/34; 20.5%). Overall time to response was known for 21 of 34 dogs (partial n=6, good n=15) with a median of 11?days (range 1 to 155). Four dogs had surgical intervention (castration n=4, cystostomy tube n=2). Three dogs were euthanased due to partial (n=1) or poor (n=2) response. Medications were discontinued in 11 of 20 (55.0%) dogs with a good response to therapy, two of these relapsed.
